Application was submitted by the Chicago Black Hills and Yellow Stone Park Association on the 10th day of August 1917 and approved on the 15th day of September 1917. Form features an image of a square with the bottom and top thirds in black and the middle third in yellow. It is signed by Commissioner of Highways Charles M. Babcock.

Application was submitted by the Winona Association of Commerce on the 7th day of August 1918 and approved on the 16th day of August 1918. Form features an image of a rectangle with the words "Yankee Doodle Highway Winona to St. Paul 'Step Lively.'" It is signed by Commissioner of Highways Charles M. Babcock.
